The Neighborhood Health Association celebrated Tuesday March 31, 2015 the launch of construction of its new $11 million health clinic � Nexus Health Care � which will be located at 14th and Jefferson streets in the UpTown neighborhood near downtown Toledo. The association was awarded a $5 million grant from the Health Resources and Services Administration, which is part of the U.S. Department of Health and Human Services, to build the facility. The rest of the construction costs, more than $6 million, are being financed through tax credits brokered by the Local Initiatives Support Corp. or LISC. 10/18/16 - Neighborhood Health Association held its grand opening for the new Nexus Health Care center downtown today with a dedication ceremony and tour of the facilities. The 43,000-square-foot facility at 1415 Jefferson Ave. offers a variety of services, including primary care, pediatrics, women�s health, and dental. It is also the new home of the Mildred Bayer Clinic for the Homeless, which moved from its previous location, also on Jefferson Avenue.

Former Use: The commercial building previously located at the property was constructed in 1921 and demolished in 2013. The Lucas County Land Bank had owned the property since December 2011. Historically the property operated as an automotive dealership and fueling location from 1921 to 2011. Two underground storage tanks were removed from the property and a Tier 1 Investigation was performed.
