Additional Property Attributes

The 4C Health Building is located at 1015 Michigan Avenue, Logansport, Indiana (subject property). The subject property consists of a 21 parcels totaling approximately 3.9 acres of land located in the City of Logansport, Indiana. Improvements on the subject property include an approximately 33,000 sq ft medical building, 15,000 sq ft of parking lot and access lanes, two (2) small sheds, and wooded land and grassy landscaping. The surrounding area is residential, industrial, and municipal in nature. The property is situated on the west side of Michigan Avenue, adjoining Logansport Memorial Hospital to the north; residential properties to the southeast and west; and an industrial property (Mathew Warren, Inc.) to the southwest. 4C Health recently moved their operations to a new location in Logansport, and the the subject property is currently unoccupied.
The Phase II ESA sampling and inspections were conducted on October 17, 2024, for the purpose of determining BER conditions associated with the building on the subject
property. The investigation included collecting and analyzing tap water samples from eight (8) drinking water taps and faucets, along with a mold inspection, and asbestos and
lead paint inspections. Tap water samples were analyzed for lead and compared to IDEM's R2 PLs and the EPA's Drinking Water action level and MCLG.

Lead was present above the R2 PL and EPA action level or the MCLG in all eight (8) initial samples but only one 2-minute sample was above the action level. This would suggest the lead is most likely leaching from plumbing within the building. Possible sources would include the fixture, natural pipe corrosion (if non-copper pipes are used) and/or the solder used in joining the pipes.

Moderate to high levels (according to MI&T) of Penicillium/Aspergillus was present in room 122. No visual indication of the presence of mold was noted in the MI&T inspection report, and the report indicated that the area appears to be localized to just that room.

ACM is present in a large room on the southwestern side of the building on the subject property. Sampling results indicate ACM to be localized just to this room.
LBP is not present in the building.
